PATH Collaborative: Contra Costa County

Our Purpose

As the selected facilitator for your Contra Costa County PATH CPI Collaborative, Intrepid Ascent is responsible for hosting and facilitating monthly convenings and working with participants to develop programmatic activities contributing to CalAIM’s whole system, person-centered approach to care. We look forward to working with our CPI participants in Contra Costa County to identify Enhanced Care Management (ECM) and Community Supports needs and gaps within the community, identify and resolve implementation issues, and monitor how Providing Access and Transforming Health (PATH) funds can be used to address implementation issues and disseminate best practices.

Currently, the Contra Costa Collaborative’s agreed upon aim statement: By December 31st, 2023 the Contra Costa County PATH CPI Collaborative will facilitate engagement between organizations capable of providing Medi-Cal services and MCPs to improve Medi-Cal standards, systems, and processes which will increase the percentage of:

  • ECM-eligible individuals enrolled in ECM services by 5%
  • Community Supports-eligible individuals enrolled in Community Supports services by 5%

Our meeting cadence is generally the third Wednesday of the month, from 9:00 to 10:30am PT
