Active PATH collaborative counties

We are engaged with regional stakeholders in California to further the Department of Health Care Services’ CalAIM PATH Collaborative Planning Implementation Initiative. The community-wide goal is to support implementation of Enhanced Care Management and Community Supports by engaging with local entities to discuss how PATH and other CalAIM funding initiatives may be used to address gaps identified in MCP Needs Assessments and Gap Filling Plans.

PATH Technical Assistance Marketplace

Intrepid Ascent is honored to have been selected by the Department of Health Care Services as a CalAIM PATH vendor. Available to providers, community-based organizations, counties, and other entities implementing Enhanced Care Management and Community Supports, the TA Marketplace is a virtual, one-stop-shop website where entities can access resources and technical assistance designed to build knowledge and capacity, implement systems and tools, advance health equity, and support cross-sector partnerships.
